2026-27 Athletic Physical Packet

The following is a list of Rank One Sport Electronic Participation Forms that must be submitted by Parent or Guardian:

• Waller ISD Athletic Pre-Participation Forms
• Special Medical Information
• Emergency Medical Information and Consent Form
• Parent/Guardian Consent
• Physical & Medical History Form (Requires medical exam - NO OTHER physical examination form will be accepted as per UIL rules. This is the ONLY form that MUST be a paper copy. It MUST be completed and returned to the athletic training staff by Wednesday, May 13, 2026.)
